Yeah, fortunately I solved this problem a while back it was two fold, 1. I was a noob putting to much pressure on my wrench when I had a false set, 2 my Tension wrench would go flying because as I corrected the false set I too preasure off the wrench and it slipped slightly down the keyway giving a small bit expossed when i re applied pressure the plug moved and the wrench went flying,
The solution is I filed down the end of the tension wrench to make it smaller then the keyway itself so even if it did slip both sides were still making contact with the keyway. I am also a lot more experiencd now, that was the big help, the cure all 100% best tool in the box
