Well, the best thing is a twisted diamond.

I know it sounds screwy, but it does actually work. Gently heat the end of a half diamond, grasp with a pair of pliers, and give the upper part of the diamond a 45 degree angle twist.

You can also do the same thing to a lifter pick, and the results are similar.

It would at first seem like you would need a left and right twist, but the fact of the matter is that you can use the twisted tip to rotate the pins left or right.

A full diamond is just too large for most of the Medeco keyways.

There is no reason why you need a left and right twist.

The twist is what allows you to "grasp" the point on the pin and turn it left or right.

There are only 5 possible rotated positions for a medeco pin (-20,-10,0,10,20 degrees) in orientation to the sidebar. It would be nice if you could some how measure these inside of the lock so you wouldn't have to rotate it 360 degrees until you find the right place :-\

They don't rotate 360 degrees.
