What makes a gonzo hook?

I've seen the term "gonzo hook" thrown about in a few forum posts and materials elsewhere. From what I can gather it's something like a medium hook. So what makes a hook a gonzo hook?

And, since a lot of people seem to point to the gonzo hook as their go-to pick, are there any manufacturers that make something close to a gonzo hook? I might be a bit of a collector...

It's certainly not a medium hook: it's an extremely long hook. It's named after (and looks not too dissimilar to) Gonzo the Muppet's nose. Personally, I've only ever used one in either really unobstructed keyways, or for really extreme high/low pin combinations.
