I wish I could figure out how post my form...
It has a signature section stating that the person will pay for the service (get the signature before service). This covers checks and C/C's. And yes, I accpet C/C-D/C, it's VERY inexpensive to do, and worth it. I don't carry more than $20-$30 personally.
I also have a section for initials stating that I am not being held liable for damage. this protects me from some idiot that's already tried to get in, messed something up, and tries to file a claim on my insurance.
I have a section for make, model, year, license, and VIN. Insurance companies want this for a reimbursement. I ask if they need the info before I write it, otherwise I just write down the year, make, and model of the vehicle.
I check ID, and match it to an insurance card or registration (for a vehicle). If the ID doesn't match a residential, I have them show me a bill. Thier name has to be on something...
I'd suggest simplifying your rates. Your form is nice, but complicated. KISS.
