Locksmith Indemnity Forms

I know many locksmiths have their customers sign an indemnity form, holding harmless the locksmith from any damages (ie, blemishes, etc) from bypassing a home or vehicle lock through means unintended by the manufacturer. Such forms are a good idea, as they protect the locksmith from claims of damage to things like door seals, that the manufacturer refuses to cover because the owner called a locksmith to "break in".

Would anyone here be willing to post a copy of their indemnity form, and we can compare different aspects of eachother's forms?

A form saying the locksmith is not liable for what happens to the lock and door?

heres the one i use on my job tickets. not perfect i know but a start (sorry for the size)

i just made that up off the top of my head one day but theres more stuff im going to add to it next time round

Quicklocks it is also worth adding that
"Locks and all items supplied or supplied and fitted by quicklocks remains the property of quicklocks until paid for in full." This at least gives you some come back if they try to get out of paying for your work.

BTW, If you'd like some help reducing the photo, so it doesn't stretch out the remaining thread, I can give you a few pointers or walkthrough.
