had to read this a couple times to wrap my head around what y'all were talking about, but I see what you are staying now..

I will admit that at first the "Drill Hole" looked strange to me, but I didn't think much of it because I thought it was a hole drilled in the face plate

So my question is, wouldn't someone have to turn the handle in the wrong direction pretty aggressively for this piece to break off... Would someone have done that in an attempt to break in ?
When I finally leave this world.. Will someone please tell my wife what I have REALLY spent on locks ...

It could have been an honest mistake from the looks of it.
A loose jacket/shirt or purse/backpack strap could have caught on that lever while someone was walking through the door and just yanked it well enough to snap that.
I looked at the pictures again and if you notice the hole is even bent inward on the 9 o clock side, and outward on the 3 o clock side, indicating that the point of stress was there and moving from left to right.

Still, I am not a professional and unless I could take that apart and find out if there's a piece of metal floating around in there I can't guarantee that's exactly what happened...it just appears like the logical explanation.
